We meet at your clients' homes, your office, other designated meeting locations, a facility, inpatient/outpatient hospitals, nursing homes, and others. In short, we come to you and your client.
Services Provided
• Medical case management: onsite care management including meeting with family members, assessment of the disabled family member, verification of medical issues, identification of problems and reporting recommendations
• Liaison between families, providers and financial and legal advisors
• Assistance with obtaining financial and social resources including application for SSI, SSDI, Medicaid, Medicare, food stamps and local agency programs
• Budgeting (monthly, annually and one-time) to allow for the set-up of housing, medical equipment and established needs
• Coordination of transportation for disabled individual, including moves
• Respite care for family members, including interviewing, selecting and overseeing in-home caregivers
• Coordination of home modifications
• One time task assignments available
• Coordination and delivery of disability planning working with all involved parties
• Strategies to maintain lifetime benefits while ensuring quality medical care
• Training of family members to provide oversight beyond services of care manager
• Monthly and annual cost analysis, current and ongoing benefit analysis with the goal of accessing 3rd party and government benefits/programs
• Advisor to legal guardians on appropriate care, medical needs and assessment of the client and family
